2015 Day Camp Authorization
The Parent/Guardian Authorization is emailed to you when we confirm your registration in summer camp. You need to print this document, complete and sign it in order for your camper to attend camp. Please bring the completed form with you on the first day of the Day Camp session.

2019 Health Exam Form
The Health Exam Form is emailed to you when we confirm your registration in summer camp. You need to print this document and have a licensed healthcare provider complete and sign it in order for your camper to attend camp. All campers must have had a health exam by a licensed healthcare provider within the 12 months preceding the start date of the camp session in which they are registered. Please bring the completed form with you on check-in day.
2019 Welcome to Camp Materials
These materials are emailed to you when we confirm your registration in summer camp. Inside you'll find tips, tricks and suggestion to help prepare you for camp, as well as a “What to Pack” list.
2019 Health History
The Health History is emailed to you for your convenience when we confirm your summer camp registration. We prefer that you provide the information requested in this document electronically by completing the “Camper Information and Health History” component of the online registration system. If you choose to print this file, please mail a copy to camp or scan it and email it to HealthHistory@camphanover.org at least 2 weeks prior to your arrival at camp. Bring the original with you to Check-in Day.
